Insurance
We accept
Medicare, Kentucky, Ohio, and West Virginia
Medicaid, and all insurances. We also participate
in most networks. However, individuals should
check with their networks to be sure.
Medicare Fact Sheet
We accept assignment.
This means that we agree to accept whatever
Medicare sets as an allowable fee as payment in
full, regardless of the amount we bill.
Medicare pays 80% of the allowable. The patient
pays 20%.
Example:
We
charge . . . . . . . . . . . . . . . . . . . . .

We will bill Medicare for you.
We need to have your signature on file in order to
do this. You signed a paper when you first
registered as a patient.
We will bill your secondary/supplemental
insurance company for you.
Most secondary carriers cover 100% of the 20% that
Medicare deems as the patient’s responsibility.
(In the above example, the secondary insurance
carrier will pay $20, and the patient pays $0.)
Some secondary carriers (many Aetna plans) cover
80% of the 20% that Medicare deems as the
patient’s responsibility.
(In the above example, the secondary insurance
carrier will pay $16, and the patient will pay
44.)
Most secondary insurance carriers do not pay
Medicare deductibles.
We will send you a bill after we receive
payment or notice of nonpayment from your
insurance carriers.
Payment plans are available. Please see our
billing representative.
Cancer Policies
If you have a cancer policy, please notify us so
that we can send you an itemized statement. We
will mail the statement to you 2-3 weeks after
completion of treatment.
